Shirts
I've picked up a possibly bad habit of buying shirts from tm lewin. They are always on sale, 5 for a dollar or something like that. They are a bit hit or miss. Some of them end up great, many so-so. Probably all made in china. I have occasionally bought nicer-quality shirts elsewhere, and liked them. I'm mulling getting some very high-quality shirts actually made for me. Not sure if it's worth it. Although my doctor did instruct me to buy better-fitting shirts.

No. She said my shirt was too big and it made me look unhealthy. I still buy shirts as if I were fat I think. I also have a neck problem, in that if I buy shirts that fit my neck, they end up too large. I guess my neck is relatively big even when I'm not being fat.
I have a similar neck problem. Traci and I went out and bought a bunch of $6 and $11 dollar shirts - Perry Ellis, Calvin Klein, etc. Many made in China, some in Sri Lanka, Bangladesh, Pakistan, or Dominican Republic. The "tents" I was wearing were ridiculously loose on me.
To Zim's comment a few weeks back, I tried to squeeze into Mediums, but could not. Larges fit my chest just fine - baggy around the waist a little bit though.
If you are rough on your shirts (food spillage, grease, pen stains, tears, etc) then getting custom made ones is unwise. However, if you get a few custom made ones for special occasions, like anniversary dinners out with the wife, etc =) ... then what harm are having a few for nice things?
I work in an office so have to wear reasonably nice shirts anyway. I did (way too late) actually discover that a nice shirt and good wool trousers not only looks better but is also much more comfortable than a cheap shirt and khakis.
my recommendation is to get a nice shirt not too pricey and get it cut down to fit you. Much cheaper than a fully made shirt and you still get the fit. I would say to find a nice Indian or Russian tailor in the area... you shouldn't have a problem finding one around you. And, from the sounds of it - you have some nice shirts that you need cut to size. Obviously go with one of your crappy shirts to the tailor until you find one that is good. :)
